I am having an issue where in some instances the toolbar button images are not displaying in one environment and display in another just fine, regardless of browser. Has anyone ever heard of this issue before?

This can happen when you are switch different styles from the (themeroller jquery UI). Either check the directory path from which you include the style or redownload the theme and included it again.
