Modal dialog box

Hello

I use standard dialog boxes for add and edit operations.

When jqModal parameter is setted to false, labels and fields are vertically aligned such as below

When jqModal parameter is setted to true, fields are centered vertically, the bottominfo also.

Here is the code :

Plz is there somebody to give me a way to obtain modal boxe with aligned fields ?
